What is a Home Care Package?

A Home Care Package is a flexible in-home support option designed to provide a wide variety of services and products for older people. There are four levels available. Each level provides a different subsidy amount that can be used to fund services to meet your care needs

  1. A Home Care Package is flexible in-home support funding designed to provide a wide variety of services. The Government offers Home Care Packages to support you to live at home. You may be considering a Home Care Package for yourself or a loved one to support you/them in activities such as the weekly shop or gardening. Your health, wellbeing, and circumstances determine eligibility for a Home Care Package.
  2. The Government requires a formal assessment of your eligibility to access a Home Care Package. You can apply for a Home Care Package through My Aged Care.   3. Packages are available in four levels of support, catering for basic needs through to more complex needs. Each level of Home Care Packages provide a different subsidy amount that can be spent on services and products that best support your independence, health and safety. With access to one of Australia’s widest networks of care workers, Care Connect will tailor flexible and independent care options to your goals and package level.

Home Care Package (HCP)

  • Level 1 - $10,271 annually
    • 1-2 hrs p/w
  • Level 2 - $18,063 annually
    • 2-4 hrs p/w
  • Level 3 - $39,310 annually
    • 4-6 hrs p/w
  • Level 4 - $ 59,953 annually
    • 8+ hrs p/w

  • With Care Connect you receive your own Care Manager.
    • A Home Care Package is structured in four different levels as everyone’s care needs are different.
  • A formal application is required through My Aged Care. You may need to wait for up to 12+ months for a Home Care Package to be assigned.
  • Your HCP level is assigned based on your circumstances.
  • Formal care plans must document individual goals as part of your HCP. All support and services must then align with personal goals in your care plan.
  • As your circumstances change it is possible to request a review for a higher level of funding.
  • Care plans are formally reviewed annually and subsidies are reviewed 1 July each year.
